I wanted to share with you what i have on my son's emergency sheet, it has all his basic health info on it, and its also a blank permission sheet for the babysitter in case there is ever an emergency when we are not around.

In my absence on ______ I authorize emergency medical treatment for Child's first and last name who is in the care of _________________________ (I leave these blank so it can be re-used each time we take him to the babysitter's)

Medical Diagnosis (if it is important, bold it, if it will not affect treatment, leave it unbolded, i noted that my son is autistic and might have trouble following directions and answering questions)

I keep it signed and in my folder. The grandparents also have a signed copy (they're usually our only babysitters) I try to keep an updated photo with it, just in case, but i'm awful and keeping current photos
