When we had an outdoor cat we didn't worm him. He always looked good and healthy. One stray I took in had worms. The OTC meds weren't strong enough so I got an Rx. Same for one of my dogs. The OTC med was fine for basic prevention/maintenance but when he was losing weight and having digestive issues, we had to get an Rx worming med. The vet said it's 10x stronger than OTC and it did the trick.
I think the recommended schedule for outdoor cats is 2-4x per year but I'm not sure.
